The Health Navigator will be involved in a variety of population health initiatives including but not limited to coordination of gap closure outreach, assistance to chronic condition focused teams, and supporting patients access community and hospital resources. This role will work with an interdisciplinary team to provide the best care possible to our patients, assist patient in navigating the health care system and overcoming barriers to care. ExperienceMinimum Required Qualifications
Must be from the community that is being served or have a familiarity of the community.
Experience in community outreach, public health, health promotion, education or health care strongly recommended.
Community Health Worker certification a plus.
